4. Enrollment Agreement Controls

6. Auto-Pay and Payment Authorization

If you enroll in auto-pay or recurring payments, you authorize Hoosier Dental Assisting School and/or its payment processor to charge the payment method you provide according to the payment schedule you accept.

You are responsible for keeping your payment method current. Failed, expired, disputed, or declined payments may result in account holds, late fees if applicable, collection activity, suspension from class participation, delayed completion documents, or other remedies allowed by your agreement and applicable law.

7. Refunds, Withdrawals, Cancellations, and Schedule Changes

Refunds, withdrawals, cancellations, missed classes, transfers, and schedule changes are governed by the written enrollment agreement, refund policy, student policies, and applicable law.

You should review all enrollment and refund documents carefully before signing or paying.

If a class is canceled, postponed, rescheduled, or modified, Hoosier Dental Assisting School will provide available options according to applicable school policy and law.

8. Attendance, Conduct, and Student Responsibilities

Students are expected to attend class, participate appropriately, complete assignments and practical requirements, follow instructor directions, comply with safety rules, maintain professionalism, and meet program standards.

Hoosier Dental Assisting School may take corrective action, suspend, dismiss, or remove a student from class or school activities if the student:

Violates school policies

Fails to meet academic, attendance, conduct, safety, or payment requirements

Submits false or incomplete information

Disrupts the learning environment

Acts in an unsafe, abusive, threatening, discriminatory, or unprofessional manner

Misuses school property, equipment, systems, or records

Violates applicable law or regulatory requirements

9. Certificates, Completion, Licensure, and Employment

Completion of a program may require attendance, participation, payment, skills performance, assignments, exams, documentation, professionalism, and other requirements.

A certificate of completion, CPR certification, radiography-related training, expanded functions training, or other credential-related support may depend on program completion, third-party requirements, state requirements, testing requirements, documentation, and applicable law.

Hoosier Dental Assisting School does not guarantee licensure, certification, employment, wages, job placement, interview offers, externship placement, or acceptance by any employer, dental office, licensing body, credentialing body, or government agency.

Any job placement, externship, interview, or employment support is provided as assistance only and does not guarantee employment.

10. Health, Safety, and Clinical Training

Students may train in or near dental office environments and may be required to follow infection control, sterilization, safety, OSHA-style, HIPAA-style, professional conduct, and school-specific rules.

Students may be required to provide vaccination, TB, or other health-related records as part of program eligibility or participation requirements.

Students must follow all safety instructions and report concerns, injuries, incidents, exposures, or unsafe conditions promptly.
